Change Of Clothes Form

When it comes to going back to school, a change of clothes is a must! Attach this change of clothes form to a gallon-size baggie and send it home for parents to fill.

Keep the change of clothes inside your classroom or in the student’s backpack to use when needed.

Be sure to send back when the season/weather changes to have appropriate clothes.
