Parent Teacher Conference Forms – Sign In Sheet, Self Reflection, Data Trackers

Take the stress out of conference prep with these parent-teacher conference forms! This resource was designed to give you everything you need before, during, and after conferences. I’ve included conference reminder slips and a sign-in form, a self-reflection sheet students can fill out to assess their own growth, goal sheets, data/progress trackers, a follow-up form, and helpful guides for writing academic, social, behavior, and personal skill goals.

What’s Included: 

Items with an asterisk* can be edited or filled out in PowerPoint or Google Slides.

• Conference Reminder Sheet.* Send home to families with the date/time of their conference.
• Conference Sign-in Sheet. Document conference attendance with this sign-in sheet.
• Self-Reflection Form. Have your students fill this form out before the conference to assess their own progress in kindergarten.
• Goal Sheet.* Prepare this editable form before the conference to tell families about their child’s individual goals.
• Conference Language Guide. If needed, you can copy the language from this guide onto the goal sheet or conference follow-up sheet.
• Sample Learning Goals. If needed, you can copy these goals onto the goal sheet or conference follow-up sheet.
• Conference Follow-up.* You can fill out this form during the conference, or you can send it home afterward with notes or follow-up on anything discussed during the conference. This form is editable so you can type, save as a PDF, and email to families. You can also print and write by hand if you prefer. I have included both a blank/printable version and an editable version.
• Progress/Data Trackers.* These fully editable documents can be used to report grades or data to parents during the conference. There are two options included that can be customized to your needs.

This resource contains a PDF with printable documents, sample learning goals, and a conference language guide.  It also contains a PPT with a fillable conference reminder sheet and editable goal sheets, data trackers, and follow-up sheets.  To edit in Google Slides, just drag the PPT into your Drive folder, open, and then save as a Google Slides presentation!
